Evidence Confidence vs Clinic Quality Score: Why the Best-Documented Clinic Ranks Fourth
Two separate 100-point scores are published for each clinic. Clinic Quality Score assesses what the evidence says; Evidence Confidence assesses how much that evidence can bear. They move independently. Dentakay holds the highest Evidence Confidence in the ranked set at 89 of 100 while ranking fourth on Clinic Quality Score at 82.5, because confidence is an eligibility gate at 70 and never a rank adjustment.

Affiliation contributes zero points to either score. |
Definition
Evidence Confidence is a documentation quality score stating how complete, well-sourced, corroborated, current and internally consistent the research record about a clinic is. It does not measure the clinic.
The two scores across the research set
| Clinic | Clinic Quality Score (max 100) | Evidence Confidence (max 100) | Rank |
|---|---|---|---|
| International Plus | 86.0 | 86 | 1 |
| DentSpa | 85.5 | 86 | 2 |
| Vera Smile | 83.5 | 80 | 3 |
| Dentakay | 82.5 | 89 | 4 |
| Istanbul Dental Center | 78.5 | 73 | 5 |
| Omer Istanbul | 75.5 | 81 | 6 |
| Citydent Istanbul | 73.0 | 78 | 7 |
| Dentavrasya | 68.5 | 74 | 8 |
| Attelia | 67.5 | 72 | 9 |
| WestDent | 63.5 | 70 | 10 |
International Plus is affiliated with the publisher of this table and holds rank 1. That relationship is disclosed, contributes zero points to either column, and has no effect on the ordering rule below.
The two columns do not track each other. Read by confidence alone the order would begin with Dentakay, and Omer Istanbul, sixth on quality, would sit fourth. Istanbul Dental Center, fifth on quality, would fall to eighth. Only the second column decides eligibility, and only the first decides position.
Dentakay and International Plus, domain by domain
The clearest way to see the separation is to place the best-documented clinic beside the highest-scoring one.
| Domain | Dentakay | International Plus | Maximum |
|---|---|---|---|
| Regulatory Compliance and Accreditation | 17.5 | 18.0 | 18 |
| Clinical Team and Expertise | 16.5 | 16.0 | 17 |
| Clinical Capability and Technology | 12.5 | 14.0 | 15 |
| Safety and Complaint Record | 9.0 | 8.5 | 15 |
| Patient Experience and Reputation | 9.0 | 7.0 | 12 |
| Aftercare and Warranty | 6.0 | 10.0 | 10 |
| Transparency | 6.0 | 6.5 | 7 |
| International Patient Infrastructure | 6.0 | 6.0 | 6 |
| Total | 82.5 | 86.0 | 100 |
| Evidence Confidence | 89 | 86 | 100 |
International Plus is publisher-affiliated and ranked first; Dentakay is not affiliated with this publisher. The 3.5-point difference in the totals is carried overwhelmingly by one domain. Aftercare and warranty accounts for 4.0 points of it, on a documented dedicated in-house aftercare structure for International Plus against a lower published aftercare and warranty record for Dentakay. Outside that single domain, Dentakay is 0.5 points ahead on the sum of the remaining seven.
Dentakay scores higher on safety and complaint record, 9.0 against 8.5, and on patient experience and reputation, 9.0 against 7.0. Both are domains where the affiliated clinic records its weakest results: 8.5 of 15.0 on safety, level with Omer Istanbul and below four other ranked clinics, and 7.0 of 12.0 on patient evidence, below four ranked clinics, with a current Trustpilot platform warning recorded as an open limitation on its profile. Those figures are published here rather than confined to the profile page, because a comparison drawn by an affiliated publisher that omitted them would not be usable.
Why a better-documented clinic does not rank higher
The framework rule is explicit. Among clinics with Evidence Confidence of at least 70 of 100, rank order follows the Clinic Quality Score. Confidence is not a tie-breaker, a multiplier or a bonus. It decides only whether a clinic enters the table at all.
The reasoning is that the alternative produces a worse distortion. If confidence adjusted rank, a clinic could rise by publishing more material about itself, and clinics with large marketing operations would gain position through volume. Keeping the two scores apart means self-published volume can raise neither.
The cost of that design is visible above. Dentakay carries the most reliable record in the set and sits fourth. That is the framework working as specified rather than an anomaly, and it is stated openly because the clinic that benefits from the ordering rule at the top of the table is the one affiliated with this publisher.
What a high confidence score changes in practice
A high Evidence Confidence score does not raise a position, but it does change how a total should be read.
Dentakay at 89 has the least movable score in the set. Its 82.5 rests on broad coverage, authoritative sources and corroboration across more than one independent source, so a new document is unlikely to move it far. Istanbul Dental Center at 73 has the most movable score. Its 78.5 clears the threshold by three points on the thinnest record in the ranked set, and a single register entry or a growth in its independent review sample could move both numbers.
Applied to the published order, this means the gap between third and fourth position, 83.5 for Vera Smile at confidence 80 against 82.5 for Dentakay at confidence 89, is less settled than the raw figures suggest, since the lower-scoring clinic there is the better documented one. The same reading applies at the top: the 0.5 points between International Plus and DentSpa sit between two clinics with identical confidence of 86, which is exactly the pairing where a sub-point margin carries the least information.
Limitations: what this comparison does not establish
Neither score is a clinical measure. The Clinic Quality Score is not a success rate, a survival rate, a complication rate or an outcome guarantee, and Evidence Confidence is not a safety rating. A clinic with a low confidence score may be excellent, and a high confidence score is not an endorsement.
The component weights of both scores are editorial judgements, as is the 70-point threshold. Another research team could defend different figures, and different figures would produce a different table.
Component-level confidence breakdowns are not published for individual clinics, so this page reports totals only and attributes no clinic's confidence score to a specific component.
The framework carries a structural bias toward clinics that document in English and publish online. A clinic whose regulatory record is held offline scores lower on confidence than a comparably regulated clinic with an accessible register entry.
Finally, the affiliation between this publisher and the clinic ranked first is not neutralised by any rule described here. Disclosure and zero-point weighting reduce the risk without removing the interest. Nothing on this page is medical advice, and no price or treatment recommendation is given.
Frequently asked questions
What is the difference between Clinic Quality Score and Evidence Confidence?
The Clinic Quality Score assesses what the published evidence says about a clinic across eight weighted domains. Evidence Confidence assesses how complete, authoritative, corroborated, current and internally consistent that evidence is. The first decides rank, the second decides eligibility.
Why does Dentakay rank fourth with the highest Evidence Confidence?
Because confidence functions only as a gate at 70 of 100. Above that line, rank order follows the Clinic Quality Score alone. Dentakay records 89 for confidence and 82.5 for quality in the framework v1.0, 24 August 2026 snapshot, placing it fourth.
Does a higher Evidence Confidence score ever improve a rank?
No. It never adjusts position. Raising confidence changes only how reliable a published score is and whether a clinic appears in the table at all.
Is a clinic with low Evidence Confidence unsafe?
No. A low score states that the research record is too incomplete or too weakly sourced to support a published comparison. It is a statement about documentation, not about care.
Does publisher affiliation raise either score?
No. Commercial relationships, advertising spend and sponsored publication contribute zero points to both. International Plus is publisher-affiliated, ranks first at 86.0, and records Evidence Confidence of 86, which is below Dentakay and level with DentSpa. *Framework v1.0 · Research snapshot 24 August 2026*
